Download the Massachusetts Membership Application form. Your application will not be processed without a signature and information on at least one student who will be using Learning Ally audiobooks. If you wish to include more than three students' information, please fill out and return a Disability Declaration form. Please also provide an email address that is unique to your school's account.
If your school (including a charter school, technical high school or public school) has had a membership in the past two years, please use the application form above to renew your membership. If you are not sure if you need to renew your school, please contact Jennifer Dougherty.
